Privacy notice
How Hermosa Glow by Nurse Guen looks after the information you give us, in plain words.

Who we are
Hermosa Glow by Nurse Guen is a clinic at 2nd Floor, JRK Arcade, Awayan, Poblacion 3, Carcar City, Cebu. We decide what information is collected and how it is used, so under the Data Privacy Act of 2012 (Republic Act No. 10173) we are responsible for it.
What we collect
- Your name, the name you prefer to be called, your mobile number and your email address.
- Your date of birth and address, when a form asks for them.
- Your bookings, what you’ve bought, and the sessions left on any package.
- Signed consent forms and the medical history on them, and the nurse’s notes about your treatment. This is health information, and we treat it with extra care.
Why we collect it
- To book your appointments and let you know about them.
- To give you treatments safely: Nurse Guen needs your medical history to know what is suitable for you.
- To keep an accurate record of what you’ve paid for and the sessions you have left.
- To keep the business records the law requires.
We only send you promotions if you’ve said yes, and you can change your mind at any time.
Who can see it
- The front desk sees your contact details, your bookings and your package balances, so they can look after your visit.
- Only the clinic’s nurses and its owner can open your consent forms, medical history and treatment notes, and every time one is opened, the system records who opened it and when.
- We don’t sell your information or share it for anyone else’s marketing. We share it only where the law requires us to.
Where it is kept, and for how long
Your records are kept in the clinic’s own system, run on secure cloud servers. Signed consent forms are stored separately, in private storage that can only be opened through the system, one form at a time, by the people named above.
We keep your records for as long as we need them to treat you and as long as the law requires for health and business records, and no longer.
Your rights
Under the Data Privacy Act, you have the right to:
- be told what we collect about you and why;
- see the information we hold about you, and have a copy of it;
- have anything wrong corrected;
- object to how we use it, or ask us to delete or block it;
- get your information in a form you can take elsewhere;
- be compensated if your information is misused;
- complain to the National Privacy Commission.
Some records, such as a signed consent form for a treatment you’ve had, we may have to keep even if you ask us to delete them. If so, we’ll tell you why.
How to ask
Call or text us on 0917 000 0000, ask at the front desk, or write to us at the clinic’s address. We’ll ask you to confirm who you are before we share or change anything, and we’ll reply as soon as we can.
This website
This website doesn’t use advertising or analytics cookies. The map on the visit page is Google Maps, which loads as you scroll to it and sets Google’s own cookies, and our Messenger, Facebook, Instagram and TikTok links take you to those services, whose own privacy policies apply.
When you book without an account, your phone keeps your name and mobile number so the next booking is filled in for you. They stay on that phone, in its browser, and “Clear” on the booking page removes them.
